Assent - Deceased Personal Representatives
- If there are any deceased personal representatives to add, you must enter each representative separately. The person’s title e.g. Mr, Mrs, Ms, Dr etc is not required. You must ensure that you enter the person’s name correctly as this is how the information will appear on the generated documentation.
- A Death Certificate and Certificate of Identity must be supplied for each deceased Personal Representative – these will be added to the Form 100E list of documents that must accompany your application.
Assent - Living Personal Representatives
- You must enter each person’s name separately. Only enter the name in this field – not the address. The person’s title e.g. Mr, Mrs, Ms, Dr etc is not required. You must ensure that you enter the person’s name correctly as this is how the information will be recorded on the generated documentation.
- If the name of the Personal Representative is different from that shown in the document, you must supply the name shown on the document. You must supply one of the following to support the difference, and this will be added to the Form 100E list of documents that must accompany your application.
